SUBJECT:  Florida – Medical Exemption

DATE:  February 20, 2001

The Department of Highway Safety and Motor Vehicles has issued their procedure for obtaining the Sun-Screening Medical Exemption Certificate.  The procedure is as follows:

1.      The person seeking the exemption must obtain form #HSMV 83390. 

2.      The applicant must complete the top portion of the form.

3.      The applicant must have his/her physician or optometrist complete the bottom portion of form HSMV 83390.

4.      The form and fees totaling six dollars and fifty-five cents must be mailed to:  Department of Highway Safety and Motor Vehicles
